Requirements requested to apply for affiliation of a center in the interior.

Documents requested:

• Copy of ID card
• Certificates and degrees
• Exequatur
• Curriculum Vitae (CV)
• Habilitation license from the Ministry of Public Health
• Photos of the center
• Doctors who work with you
  • Copy of ID card
  • Curriculum Vitae (CV)
  • Exequatur of each doctor

Note:

The Center must be located in the interior of the country, otherwise it will not be able to be affiliated.

In exchange for the affiliation, the Dental Center will have the benefit of accepting the following ARS through Amerident.
